LE PRIEURÉ NURSING PRACTICE

The practice nurses

Based in the Prieuré’s outbuildings since November 2023, the practice welcomes patients by appointment and arranges home nursing visits.

Antoine GRAVIERRegistered nurse · Practice owner
Sébastien LAPLANTERegistered nurse · Collaborating nurse

One number to reach the nurse providing your care.

The practice welcomes babies, children, adults and older people.

Antoine speaks some English. Professional telephone interpreting can be arranged through the URPS, under the ARS-funded scheme. Please tell us which language you need when you first contact us.

After a hospital stay

New patients can contact the practice after hospital discharge, with or without technical nursing needs. An initial discussion, which may be by telephone, helps organise care. Tell us your discharge date, prescriptions and chosen pharmacy or equipment provider so we can coordinate with them.

Equipment for your care

Vein finder, infusion pump, syringe driver and portable automated defibrillator.

French health insurance. Third-party payment.

Direct billing is available for the French statutory insurance share and most French complementary insurers. Cards, cheques and cash are accepted. Cover depends on your entitlement and prescribed care.

No French health cover, or foreign insurance? Call before your appointment to discuss the arrangements.

01

Your prescription

Depending on the care required.

Common questions

How can we help?

Death certificates

Antoine GRAVIER is trained, authorised and registered on the list of nurses permitted to issue death certificates in France. He may be contacted by SAMU medical dispatch and attend, subject to availability, an adult death at home, including hospital-at-home care (HAD), or in an institution, within the legal framework.

This does not cover deaths of minors, violent or suspicious deaths, or deaths on public roads or in places open to the public. A doctor or the SAMU emergency medical service must be contacted in these situations. Medical expertise is required if the nurse cannot determine the cause of death alone.

Working with your other care providers

The practice works with hospital-at-home services (HAD), home nursing services (SSIAD), mobile palliative care teams (EMSP) and DAC21.

It provides care in Côte-d’Or and Yonne. The HAD service and mobile palliative care team involved depend in particular on the patient’s municipality.

The practice is a member of CPTS Auxois Morvan.

You are free to choose your nursing practice, doctor, pharmacy and medical equipment provider.

DAC21 · Care coordination

DAC21 supports complex care situations involving illness, loss of independence or social difficulties, at any age. It helps assess needs, identify services and coordinate professionals.

For patients and relatives, this can make discharge home, access to support and ongoing care easier to organise, alongside the usual care team.

DAC21 website
CPTS Auxois Morvan

The CPTS Auxois Morvan brings local professionals together to improve access to a regular doctor and unscheduled care. It develops coordinated pathways, including older people, heart failure and chronic kidney disease.

Its prevention work includes health education, HPV vaccination and screening. Current local programmes are described on its website.

About the CPTS
HAD · Hospital at home

Hospital at home provides hospital-level treatment in a familiar setting when appropriate, including complex dressings, intravenous treatments or palliative care.

It requires a prescription, assessment and the person’s agreement. The service coordinates staff and equipment and provides continuity of care around the clock.

About hospital at home — Service Public
SSIAD · Home nursing services

SSIAD services provide prescribed nursing, personal hygiene and comfort care for older people and eligible younger people with disabilities or chronic illness. They help preserve independence and dignity at home.

Admission depends on assessed needs, the service area and available places. The transition to SAD services brings daily support and care closer together.

About SSIAD and SAD — CNSA
EMSP · Mobile palliative care teams

Mobile palliative teams help improve the patient’s comfort, wellbeing and quality of life through specialist assessment of pain, distressing symptoms and psychological needs, while respecting the person’s wishes.

Support can begin alongside treatment of the illness. They also help relatives and advise the usual care team; home visits depend on local arrangements.
